How to reduce AI hallucinations

Reduce unsupported claims through source boundaries, uncertainty, and independent checks.

Limit what the answer may rely on

For a factual brief, supply dated sources and ask the assistant to use only those materials. A bounded evidence set makes checking easier. It does not make the output automatically true: the assistant can still misread, omit, or combine details incorrectly.

Make gaps visible

Request “not provided” for missing details and separate facts from interpretation. Avoid prompts that demand a confident answer when evidence is absent. Ask for unresolved questions rather than fabricated completeness.

Audit claims, not confidence

Extract names, dates, quantities, and causal assertions into a checklist. Open the cited source and find the passage supporting each claim. A working link alone is not enough; the page must support the statement being made.

Use a human stop rule

Do not publish an unresolved factual claim merely because a second AI response repeats it. Remove it, narrow it to what the evidence supports, or obtain another reliable source. For consequential decisions, use appropriate expert review.

Try it on your own work

Run a claim audit on a draft and independently verify three factual statements before revising it.
